Dear Colleagues,

I trust this post finds you all very well!

I would be grateful if you could please provide (based on your experiences), the possible sources (misuse, VVM, expiry, breakage, etc.) and rate of occurrence of close vial vaccine wastage at the lowest distribution and service delivery levels?

Dear Hamadou

We are administering Vccines provided by the Government as per National Immunization Schedule-Karnataka state and Dakshinakannada district specific (e.g. J.E vaccine is not applicable to our district and Rota is yet to be introduced in the state). All thes vaccines are provided with VVM. 

If you have any study prtocol, and if you can share, we may be able do the study in our vaccination clinic and provide valuable information.

Optional vaccines applicable to India, [Pneumococcal, Rota, Meningococcal, Seasonal Influenza, Varicella, Typhoid, MMR, HepA, HepB monovalent, HPV, any commercially available penta/quadri/tripple have no VVM. 

On demand we do administer these vaccines to the benficiaries of affordable family with information that the potency of the vaccine is not known as they are not dressed with VVM.

That's a great question, Hamadou. And data that is notoriously difficult to obtain. One experience I'm familiar with is Mozambique, where a very low number of closed vials wasted gets reported--normally less than 10 vials per province over a year. Of course, there may be incidents of higher closed vial wastage due to expiry or a broken fridge, but on a regular basis, reporting is very low. Reality, of course, may be different than the reports reflect, as we know health workers may be hesitant to report closed vial wastage as it may be seen as their fault.
