I have had similar experiences. The countries I have worked in during the last few years as a consultant, do not appear to have sufficient funds for proper implementation of routine immunization, particularly for training, supervison, spare parts etc., including funds for transport to take supervisors around. Most countries depend entirely on UNICEF and WHO for funding of activities related to routine immunization, and UNICEF and WHO are experiencing cuts in contributions from donors. Only one example: in more than one country I have experienced that district immunization officers very much looked forward to accompanying me on field trips in their district. There is simply no funds for them to travel themselves, so travelling in the official car at my disposal is a welcome opportunity to visits the clinics in their districts. Also, it is not rare to find examples of countries where training in basic EPI knowledge, has not been conducted for years. Last: what will happen to those countries that do not reach the goals, to which they have committed themselves in order to receive a serie of GAVI fund payments? Will GAVI funding be stopped in their case? If so, what then about the many children that will not be immunized. This question was raised at the Technet meeting in Delhi, but no reply was given. Now I see that many countries have not been able to deliver on their promises.
