In my experience with Afghanistan and Pakistan where the ambient temperature is at extremely high and low, adjusting the ILRs thermostat to a lower power setting may help but still does not completely eliminate the problem of vaccine freezing. Each setting of the thermostat has a different range of cut off and cut in temperatures of the refrigerant compressor for which only the manufacturer knows about. We already knew that all EPI vaccines at the facility or district level which uses ILRs have to kept at +2° to +8° Celsius. I think it would be very helpful if the manufacturer of the vaccine ILRs would consider to redesign the refrigeration system including the thermostat. It would be easy for the service provider if the ILR thermostat has just an on and off position that is to cut off the compressor at +2° Celsius and on the compressor at +8° Celsius at varied ambient temperatures and maximum allowable heat loads from the vaccines and ice packs. How about ice pack freezing for outreach vaccination. This could be handled with a unit with both refrigerator compartment and freezer compartment. To freeze the required ice packs the freezer compartment should have a separate refrigeration system and a thermostat which does affect the hold over time of the ILR. Alejo H.
