1.2 Mini-mPIE Health Facility Questionnaire (Appendix 2)
While EPI and NMP staff in each country are expected to carry out routine monitoring and supportive supervision as part of the introduction process, this tool is proposed for use as part of the malaria vaccine mini post-introduction evaluation (mini-mPIE), recommended within the first 3 - 6 months of introduction. Available documentation from supportive supervision should be used to complement data collected with this tool.
